Abstract

Ketamine misuse is the most common illicit drug used in Singapore, Hong Kong and areas of China and Taiwan. A Chinese American male presented to our inpatient rehabilitation unit with genital urinary complaints, of hematuria, dysuria and frequency. He had been misusing ketamine for many years and had similar complaints in the past. Healthcare providers should be aware that ketamine user disorder may present primarily with genital urinary complaints. Cessation of ketamine use is of upmost importance since chronic irreversible fibrosis of the genital urinary tract may occur with continued misuse.

Introduction

Ketamine hydrochloride, a noncompetitive N-Methyl-D-Asparate (NMDA) receptor antagonist, was initially introduced in the 1960’s as a dissociative anesthetic [1]. Ketamine noncompetitively binds to the PCP receptor and inhibits glutamate activation of the NMDA channel [2]. In recent years it has been studied for its rapid antidepressant effects, particularly with treatment-resistant depression patients.

In addition, ketamine has been frequently used recreationally since the 1980’s, particularly in the nightlife and club environment [2]. Also known by its street name “Special K,” the drug can be sniffed, smoked, taken PO, or injected. Users of the drug experience decreased inhibitions and a dissociative mental state [2]. At high doses, users experience nausea, increased heart rate and blood pressure, and potentially a cataleptic-like, “out-of-body” or “near-death” experience, which is referred to among users as a “K-hole” [3].

Ketamine use has increased dramatically in the last decade, reaching a worldwide prevalence of 5.7 percent of all drug users [4]. Pal et al. (2013) found that 18.7 percent of drug users in the United States have reported lifetime use of ketamine [5], and 5.8 percent had used it recently. Misuse of ketamine is particularly widespread in Southeast Asian countries, and it is the single most abused illicit drug in Hong Kong [6].

Habitual ketamine use is also associated with genitourinary symptoms, such as dysuria, debilitating urinary frequency, urgency, urge incontinence and hematuria [7]. One of the most commonly associated symptoms is ulcerative cystitis [1,8,9]. The incidence of lower urinary tract symptoms in ketamine misusing patients is approximately 30 percent, and in general, affected patients tend to be young with a peak age range of 16 to 35 years [10]. Ketamine-induced cystitis was only first identified in 2007 [1], and the pathophysiology has yet to be completely elucidated [9].

Case Report

A 24 year-old Chinese-American heterosexual male was admitted to an inpatient addiction rehabilitation program for the treatment of intranasal ketamine use disorder. The patient’s mother died when he was 21 years old, and he lives at home with his father and sister. He has smoked a pack of cigarettes per day since age 17. His psychiatric history is unremarkable.

The patient reported daily intranasal ketamine use beginning at age 20. He has since had several failed attempts of abstinence. His drug use is strictly limited to ketamine, and he has not combined ketamine with any other illicit or prescribed controlled prescription medications. On admission, the patient reported several urinary complaints, which included frequency, urgency, dysuria, gross hematuria, and suprapubic pain. His physical examination was within normal limits, with the exception of moderate suprapubic pain on palpation. There were no genital lesions, no inguinal adenopathy, and no penile discharge. His medical history is otherwise negative.

On laboratory examination, the complete blood count and comprehensive metabolic profile were within normal limits. Routine urinalysis revealed gross hematuria, with a red blood cell of >182 per high power field, and a white blood cell count of 96 per high power field. Chlamydia trachomatis and gonococcal DNA, SDA test, syphilis serology, and HIV antibody were all negative. Urine immunoassay toxicology was negative for opiates, cocaine, methadone, amphetamine, THC, and benzodiazepines. Urine drug toxicology was not performed for ketamine.

The patient was treated symptomatically with NSAIDS and acetaminophen. Oxybutynin, an anticholinergic antispasmodic at 5 mg PO TID was added to the regimen. His urinary signs and symptoms gradually improved during his three-week admission. He was advised to remain totally abstinent from ketamine, and informed that continued use would lead to increasing urinary tract complications.

Discussion

This patient presented clinically with typical signs and symptoms of ketamine-associated cystitis. The patient was a long-term regular user of ketamine and had the classic signs and symptoms of urinary tract complications.

Ketamine-associated cystitis was first identified in 2007 [1]. The incidence of lower urinary tract symptoms in ketamine misusing patients is approximately 30 percent [10]. Males and females are equally affected [8].

Studies have suggested that chronic ketamine use may induce an autoimmune or inflammatory response in the bladder wall [8,9], neovascularization and petechial hemorrhage [7], as well as complications involving ureteral wall thickening, and irregular bladder mucosa and hydronephrosis [11,12]. A recent study showed that ketamine-induced ulcerative cystitis and bladder apoptosis involves oxidative stress mediated by mitochondria and the endoplasmic reticulum [13].

Failure to recognize and delayed diagnosis of ketamine-induced cystitis may result in irreversible renal tract damage, requiring surgical intervention [12], and/or resulting in renal failure requiring dialysis [8]. Urinary tract protective agents such as chondroitin sulfate, pentosanpolysulfate, and hyaluronic acid have been shown to result in some short-term benefit [8,10]. Complete and timely abstinence from ketamine is the most effective strategy for preventing ongoing ketamine cystitis and urinary tract damage [14].

Should Pharmacological Treatment for Adolescents with Opioid Dependence Differ from that for Adults?

T he 17-year-old female patient presented herself for the first planned inward treatment. She reported having smoked Cannabis since the age of 13 years. At the age of 15 she smoked Heroin for the first time, and she did so (2 to 3 g daily) until the beginning of her substitution treatment five months ago with 80 mg L-Polamidon which she received from a general practitioner. In contact cautious, visibly excited and a sad basic mood she wished a detoxification treatment and subsequent psychotherapy. She was born in Iran. The father was a professional bodybuilder and was murdered in 2002 there. The 45-year-old mother had a fitness club in Iran. The patient immigrated to Turkey at the age of ten together with her mom and 18-year-old sister. Her mother and her sister still lived in Turkey. She reported in good English about the separation from her mother after beginning to work with 13 years. At the age of 15, her uncle from Iran met her and lured her into an apartment. Against her will he married her with his son, her cousin. He mistreated her and after month she had succeeded in escaping, come to Germany via Greece. Living in a girls’ home for five months she was attending school, showing good achievements and liked to be an ergotherapist, because she love to work with her hands, paint and do yoga.

Waterpipe Smoking: Gateway to or Escape from Cigarette Use among Current Waterpipe Users in Riyadh, Saudi Arabia

Introduction: The purpose of this study is to determine whether waterpipe use may contribute to the onset of cigarette use or if it may help current cigarette smokers reduce or quit cigarette smoking.

Method: This is a cross-sectional study of 622 current waterpipe smokers selected randomly from within 15 different randomly selected waterpipe lounges in Riyadh, Saudi Arabia. Using an iPad volunteers completed a self-administered survey regarding their smoking history, tobacco withdrawal symptoms, and demographics. Those who initially started smoking with waterpipe and those who initially started smoking with cigarettes were analyzed separately.

Results: Logistic regression showed that both the years of waterpipe use and withdrawal symptoms significantly predicted being a current cigarette smoker (waterpipe starters: years of waterpipe use: OR=1.18, C.I.=[1.07-1.30] withdrawal symptoms: OR=1.11, C.I.=[1.01-1.22] and cigarette starters: years of waterpipe use: OR=1.08, C.I.=[1.00-1.17] withdrawal symptoms: OR=1.15, C.I.=[1.03-1.27]. However, the likelihood of being a current cigarette smoker decreased as age and frequency of waterpipe use increased. Additionally, there was evidence that waterpipe withdrawal symptoms were predicted by frequency of waterpipe use.

Conclusions: Waterpipe use may be a potential gateway to cigarette use. However, among those who initially started smoking with cigarettes, older age and greater frequency of waterpipe may protect against cigarette use.

Implications: The study findings add additional evidence that smoking waterpipe may lead to initiate another smoking behavior, in a particular cigarette use, especially among young individuals. However, long history and greater use of waterpipe could be associated with less likelihood of being a current cigarette user. Health promotion and intervention programs must consider the mutual association between waterpipe and cigarette use among youth and young adults in order to develop effective health promotion programs.

Beverage-Specific Alcohol Sales and Gender Difference in Life Expectancy in Russia

Background: The difference in life expectancy at birth between males and females in Russia is unprecedently high compared with developed countries.

Objective: The aim of the current study was to examine the aggregate-level relationship between the consumption of different alcoholic beverages and gender difference in life expectancy in Russia between 1970 and 2015.

Method: To estimate the relation between these variables across the study period a time series analysis was performed using the statistical package “Statistica 12. Stat Soft”.

Results: The results of ARIMA analysis indicate that vodka sales are closely linked with gender difference in life expectancy in Russia: an additional liter of vodka sales per capita was estimated to increase in the difference between male and female life expectancy by 4.1%. The results of the analysis also suggest that 18.5% of the difference in life expectancy between males and females in Russia could be attributed to consumption of vodka.

Conclusions: This piece of evidence provides support for the hypothesis that high level of vodka consumption in conjunction with binge drinking pattern may be a major reason for the high gender difference in life expectancy and its dramatic fluctuations in Russia during the last few decades

Addressing the High Prevalence of Smokeless Tobacco Use in Pastoralist Communities of Ethiopia

The use of smokeless tobacco, which includes snuff and chewing tobacco, is low in majority of the countries when examined for both women and men. In general, the use of smokeless tobacco among men is more common in South and Southeast Asia than in any other region. Yet large portions of the Borena’s adult pastoral communities are smokeless tobacco users. It is needed a call to action for policymakers and health professionals to improve the health and well being of pastoral communities of Ethiopia’s adults by increasing their access to anti notice information on smokeless tobacco use.

Performance Measurement in Behavioral Healthcare Centers

Behavioral Healthcare Centers often find themselves frustrated by their lack of success in defining and meeting their key organizational objectives. This frustration is not the result of lack of skill or passion, but rather lack of clear organizational goals and concrete measurements. Executives must answer the question as to why their organization exists in the first place. What is the mission that will drive all other decisions in the organization? This mission should not be so generic that it could fit every other organization in your industry. Rather, it should help everyone in the agency understand what makes it unique.

Drug Abuse among Street Children

In The Gambia like all nations, drug abuse is seen as a social and health problem that has many serious implications for the physical, social, psychological and intellectual development of the victims more especially, the children. Therefore, it continues to be a concern to families, community leaders, educators, social workers, health care professionals, academics, government and its development partners. Though there some studies on drug abuse, there is none on children and drug abuse focusing on the street children the most vulnerable category. Street children are hypothesized to be more at risk of any epidemic including drug abuse. This study sought to determine the risk and prevalence of drug abuse among street children focusing on those in Praveen, the car parks. The research was focused on six critical areas: level of knowledge of drug abuse, perception towards it, level of knowledge of the causes of it in the community and among street children, level of knowledge of negative impacts of it, level of knowledge of the preventive methods; and level of knowledge of the support services and treatments needed by victims. A structured questionnaire was used to collect the data from thirty five participants (i.e. one driver and six casual apprentices from each of the five car parks) were interviewed. The data was presented and analyzed using tables and percentage. The findings revealed among other things, that there is high level of awareness of drug abuse but the feelings towards it is mixed. Like other children, street children are abusing drugs mainly due to peer influence with the ultimate objective of getting high to relief stress, group recognition, trusted by peers, etc. Similarly, participants are highly aware of the negative impacts encompassing f ighting, stealing, mental illness, etc. To finance the behaviour, victims are engaged in all types of dangerous antisocial behaviour including romantic ones exposing them to a range of diseases including STIs and HIV/AIDS. Marijuana is the most commonly abused drug. Though in the minority, some have started experimenting cocaine/ coke, hashish; and heroin. While participants have good knowledge of the critical methods to fight drug abuse, the support services needed by victims, victims are mostly reluctant to seek the services not only because they are hard to find but fear societal stigmatization, exclusion and discrimination and professionals’ maltreatments.
